NSAID

NSAID ย่อมาจากคําว่า Nonsteroidal anti-inflammatory drug หมายถึง ยาต้านการอักเสบชนิดที่
ไม่ใช่สเตียรอยด์เป็นยากลุ่มที่ใช้เป็นยาแก้ปวดได้ดีโดยเฉพาะ อาการปวดจากการอักเสบ ยาที่จัด
เป็นนแม่แบบ คือ แอสไพริน (Aspirin) ซึ่งเป็นที่รู้จักกันดีและใช้กันมานาน

ยาลดการอักเสบหรือยาแก้ปวดกลุ่ม NSIAD ได้มีการใช้การมานาน และปัจจุบันก็มียาอยู่ในตลาดมากมายมาย ทำให้ผู้บริโภคไม่สามารถบอกได้ว่ายาตัวไหนดีที่สุด เนื่องจากว่าการตอบสนองของยาแต่ละคนและแต่ละขนาดของยาจะไม่เหมือนกัน ดังนั้นผู้บริโภคควรจะมีความรู้ในการเลือกใช้ยา

กลไกการออกฤทธิ์ของยากลุ่ม NSAID

การอักเสบของอวัยวะในร่างกายเกิดจากการที่เซลล์หลังสาร ที่เรียกว่า พรอสตาแกลนดิน (Prostaglandin) ซึ่งเป็นสารสื่อกลางในการกระตุ้นทำให้เกิดอักเสบ ยากลุ่ม NSAIDs นี้ออกฤทธิ์โดยการยับยั้ง enzymes ที่เรียกว่า cyclooxygenases ทำให้การหลั่งของสาร พรอสตาแกลนดิน (Prostaglandin) ลดลงทำให้การอักเสบลดลง และลดอาการปวด การยับยั้ง enzyme ดังกล่าวทำให้เกิดผลข้างเคียงตามมาอีกหลายประการ

ชนิดของยา NSAID

แบ่งออกเป็นสองชนิดคือ

Nonselective NSAIDs หมายถึงยาจะยับยั้ง enZymes ทั้งชนิด COX-1 and COX-2 enzymes ซึ่งการที่มันยับยั้ง enzyme ทั้งสองชนิดทำให้ยากลุ่มนี้ระคายต่อกระเพาะอาหาร ยากลุ่มนี้ได้แก่ aspirin, ibuprofen naproxen diclofenac
Selective NSAIDs ยากลุ่มนี้จะเลือกยับยั้งเฉพาะชนิด COX-2 enzymes ดังนั้นจะระคายกระเพาะน้อยกว่ายาในกลุ่มแรก ดังนั้นผู้ป่วยซึ่งมีประวัติโรคกระเพาะควรจะได้รับยาในกลุ่มนี้ ยาในกลุ่มนี้ได้แก่ Celecoxib

ดังนั้นผู้ป่วยที่เป็นโรคหลอดเลือดหัวใจตีบ หรือเคยทำballon หลอดเลือดหัวใจ หรือเคยเป็นอัมพาตหรืออัมพฤต หรือผู้ป่วยที่มีความเสี่ยงต่อโรคหัวใจสูงควรจะหลีกเลี่ยงยา NSAID โดยเฉพาะยาในกลุ่ม Selective NSAIDs สำหรับผู้ที่เป็นโรคกระเพาะอาหาร หรือเคยมีเลือดออกมาควรจะหลีกเลี่ยงยาในกลุ่ม Nonselective NSAIDs
ผู้ป่วยที่มีโรคไต โรคหัวใจวาย โรคตับ ความจะหลีกเลี่ยงยาแก้ปวดกลุ่มนี้

ข้อบ่งชี้ในการใช้ยา

ยากลุ่มนี้ใช้ในการรักษาอาการปวดที่เกิดจากการอักเสบโรคหรือภาวะที่นิยมใช้ยานี้ได้แก่

ยานี้มีผลข้างเคียงที่ผู้รับประทานจะต้องทราบ

ปัจจัยเสี่ยงต่อโรคหัวใจและหลอดเลือด

ยากลุ่ม NSAIDs จะเพิ่มความเสี่ยงในการเกิดโรคกล้ามเนื้อหัวใจขาดเลือด โรคหลอดเลือดสมอง ความเสี่ยงนี้จะเพิ่มขึ้นเมื่อใช้ยานี้เป็นเวลานาน และมีปัจจัยเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือด
ยา NSAIDs ห้ามระงับอาการปวดหลังผ่าตัดหลอดเลือดหัวใจ

ปัจจัยเสี่ยงต่อระบบทางเดินอาหาร

ยากลุ่ม NSAIDs จะเพิ่มความเสี่ยงในการเกิดโรคแทรกซ้อนทางเดินอาหารได้แก่ เลือดออกทางเดินอาหาร แผลกระเพาะอาหาร กระเพาะทะลุ

อาการข้างเคียงของยา

ผลข้างเคียงของยา NSAIDs ที่อันตรายให้หยุดยาทันทีและปรึกษาแพทย์ในกรณีที่พบอาการดังต่อไปนี้

มีอาการแน่หน้าอก อ่อนแรง หายใจเหนื่อยหอบ อาการเตือนของสมองขาดเลือด
อาเจียนเป็นเลือด ถ่ายอุจาระดำ
ไอเสมหะมีเลือดปน อาเจียนดำเหมือนน้ำโค๊ก
บวมเท้า หรือน้ำหนักขึ้นมาก
ปัสสาวะน้อยกว่าปกติ
คลื่นไส้อาเจียน แน่นชายโครง เบื่ออาหาร ปัสสาวะเข้ม อุจาระซีด ตัวเหลืองตาเหลือง
มีผื่น เลือดออกง่ายกล้ามเนื้ออ่อนแรง
เกิดอาการแพ้ชนิดรุนแรงได้แก่ ไข้ เจ็บคอ บวมใบหน้าหนังตา ผื่นลอกตามผิวหนัง ได้ Steven Johnson's syndrome,Erythema multiforme

ผลข้างเคียงของยากลุ่มนี้
คนส่วนใหญ่จะรับยานี้ได้อย่างปลอดภัย นอกจากบางคนที่เป็นกลุ่มเสี่ยงอาจจะมีผลข้างเคียงของยา ผลข้างเคียงที่พบได้บ่อยได้แก่

ระคายต่อกระเพาะ หากรับประทานขนาดยาไม่สูงและรับช่วงสั้นๆอาจจะมีอาการแน่นท้อง เสียดท้อง หากรับประทานยาในขนาดสูงและติดต่อกันเป็นเวลานานก็อาจจะทำให้เกิดโรคกระเพาะ และมีเลือดออกได้
ตับอักเสบ หากรับประทานยากลุ่มนี้ในขนาดสูงเป็นระยะเวลานาน
ไต การใช้ยากลุ่มนี้แม้ว่าในระยะสั้นก็อาจจะทำให้ความดันโลหิตสูงขึ้น และมีปัญหากับไต ดังนั้นคงต้องติดตามความดันและการทำงานของไต
หูอื้อ พบมากในผู้ที่รับประทานยาในขนาดสูง
มึนงง เวียนศีรษะ ปวดศีรษะ
น้ำมูกไหล เจ็บคอ
ผื่นคันเล็กน้อย

ก่อนรับประทานยาจะต้องแจ้งแพทย์อะไรบ้าง

ไม่ควรใช้ยานี้ก่อนหรือหลังการผ่าตัดเบียงเบียน หลอดเลือดหัวใจ เนื่องจากยานี้จะเพิ่มความเสี่ยงของการเกิดโรคหัวใจ และโรคหลอดเลือดสมอง
ยานี้จะเพิ่มความเสี่ยงของการเกิดเลือดออกใน กระเพาะอาหาร และกระเพาะอาหารทุลุ ดังนั้นจะต้องระวังในผู้ป่วยสูงอายุ และผู้ที่เคยเป็นโรคนี้มาก่อน

ดังนั้นการใช้ยานี้อย่างปลอดภัยจะต้องแจ้งอาการดังต่อไปนี้ให้แพทย์ทราบ

แพ้ยา หากท่านผู้อ่านแพ้ยาดังต่อไปนี้ควรแจ้งแพทย์หรือเภสัชก่อนใช้ยา และไม่ควรใช้ยาในกลุ่มนี้เองเนื่องจากอาจจะแพ้ยา

Aspirin or other salicylates
Ketorolac
Oxyphenbutazone
ibuprofen (e.g., Suprol)

หากท่านผู้อ่านเป็นโรคหัวใจหรือโรคไตไม่สามารถรับ อาหารที่มีเกลือมากไม่ได้ ต้องแจ้งแพทย์เนื่องจากยาในกลุ่มนี้บางตัวมีเกลือผสมอยู่ อาจจะทำให้โรคที่ท่านเป็นอยู่มีอาการทรุดลง
ประวัติการเกิดโรคหัวใจ โรคหลอดเลือดสมอง โรคหัวใจ หัวใจวาย ความดันโลหิตสูง
ประวัติโรคกระเพาะอาหาร หรือเคยมีเลือดออกทางเดินอาหาร
การตั้งครรภ์ เนื่องจากการใช้ยาในคนท้องอาจจะทำให้เกิดโรคแทรกซ้อนขณะตั้งครรภ์ และอาจจะเกิดปัญหากับทารกดังนั้นจึงไม่ควรซื้อยารับประทานเองขณะตั้งครรภ์
ขณะให้นมบุตร ไม่ควรใช้ยาดังต่อไปนี้ indomethacin meclofenamate phenylbutazone piroxicam เนื่องจากมีรายงานว่าเกิดผลข้างเคียง
ผู้สูงอายุอาจจะทำให้สับสน หน้า เท้าบวม ปัสสาวะออกน้อย
หากท่านผู้อ่านที่รับประทานยาอื่นร่วมด้วยต้องแจ้งแพทย์และเภสัชทุกครั้ง
ถ้าหากท่านมีโรคประจำตัวต้องแจ้งทุกครั้ง เช่น

โรคหัวใจ
โรคกระเพาะอาหาร
โรคตับ
โรคไต
โรคลมชัก
โรคหอบหืด
โรคเลือดมีประวัติเลือดออกง่าย
ริดสีดวงจมูก

ความปลอดภัยสำหรับคนตั้งครรภ์

จัดอยู่ในกลุ่ม D ไม่ควรให้ผู้ที่ตั้งครรภ์ไตรมาศสุดท้ายเพราะอาจจะมีผลเสียกับทารก สำหรับผู้ที่เลี้ยงลูกด้วยนมแม่ต้องปรึกษาแพทย์ก่อนรับประทานยานี้เด็กอายุ น้อยกว่า 2 ปีไม่ควรจะใช้ยานี้

การใช้ยา NSAID ในภาวะต่างๆกัน
ผู้ที่เป็นโรคหัวใจหรือมีความเสี่ยงต่อโรคหัวใจ
ผู้ที่เป็นโรคหลอดเลือดหัวใจตีบ เจ็บหน้าอก หรือมีความเสี่ยงต่อโรคหัวใจขาดเลือด หรือโรคอัมพาต ควรจะหลีกเลี่ยงยาในกลุ่มนี้ หากจำเป็นต้องรับยาในกลุ่มนี้ต้องรับประทานยาในขนาดต่ำสุดและระยะสั้นที่สุด สำหรับ aspirin ใช้เป็นยาเพื่อลดการตีบของหลอดเลือด
โรคกระเพาะอาหาร
ผู้ที่มีโรคกระเพาะอาหารหรือเคยมีเลือดออกในทางเดิน อาหารจะเสี่ยงต่อการมีเลือดออกหารับประทานยาแก้ปวดกลุ่มนี้ แนวทางแก้ไขอาจจะลดปัญหาเรื่องโรคกระเพาะดดยการรับประทานยาลดกรด เช่น famotidine omeprazole lansoprazole ร่วมด้วยจะช่วยลดการเกิดโรคกระเพาะ
ภาวะเลือดออก
หากจะผ่าตัดต้องหยุดยากลุ่มนี้เป็นเวลา 7 วันก่อนผ่าตัด
ภาวะบวมน้ำ
ผู้ป่วยที่มีแนวโน้มจะบวม เช่นผู้ป่วยโรคหัวใจวาย โรคตับ และโรคไตเมื่อรับประทานยานี้จะทำให้เกิดการบวมซึ่งอาจจะทำให้โรคกำเริบ
โรคไต
ยาแก้ป่วยกลุ่มนี้อาจจะทำให้การทำงานของไตแย่ลง ดังนั้นไม่แนะนำให้ใช้ยากลุ่มนี้ในผู้ป่วยโรคไต
ผู้ป่วยตั้งครรภ์หรือเลี้ยงลูกด้วยนม
ไม่แนะนำให้ใช้ยากลุ่มนี้ในการตั้งครรภ์ไตรมาศสามเพราะอาจจะเกิดโรคแทรกซ้อนกับเด็ก แต่สำหรับระหว่างการให้นมสามารถให้ยานี้ได้
ผู้ป่วยที่แพ้ aspirin
หากมีประวัติแพ้ยาก็ควรจะหลีกเลี่ยงยาในกลุ่มนี้ทั้งกลุ่มเพราะอาจจะมีอาการแพ้ยาได้
ยาลดการอักเสบ NSAID
ยาแก้อักเสบหรือยาแก้ปวดที่แพทย์และผู้ป่วยนิยมใช้กันใช้ทั้งลดไข้ในเด็ก ลดอาการข้ออักเสบ แต่ยากลุ่มนี้ก็มีผลข้างเคียงหากใช้ยาไม่ถูกต้อง ท่านผู้อ่านที่ใช้ยากลุ่มนี้ต้องศึกษาถึงผลดีและผลเสียของยากลุ่มนี้ข้อบ่ง ชี้ในการใช้ยากลุ่มนี้ได้แก่

ใช้เป็นยาแก้ปวด Analgesic ได้แก่ยา Diclofenac; Fenoprofen; Floctafenine; Ibuprofen; Ketoprofen; Meclofenamate; Mefenamic Acid; Naproxen
ใช้ลดการอักเสบ Anti-inflammatory ได้แก่ยา Flurbiprofen; Indomethacin; Naproxen; Sulindac; Tenoxicam
ใช้แก้ปวดประจำเดือน Antidysmenorrheal ได้แก่ยา Diclofenac; Flurbiprofen; Ibuprofen; Indomethacin; Ketoprofen; Meclofenamate; Mefenamic Acid; Naproxen; Piroxicam
ใช้รักษาโรคเก๊า Antigout agentไดแก่ยา Diclofenac Fenoprofen; Floctafenine; Ibuprofen; Indomethacin; Ketoprofen; Naproxen; Phenylbutazone; Piroxicam; Sulindac
ใช้ลดไข้ Antipyretic—Ibuprofen; Indomethacin; Naproxen
ใช้รักษาข้ออักเสบ Antirheumatic ได้แก่ยา Diclofenac; Diflunisal; Etodolac; Fenoprofen; Flurbiprofen; Ibuprofen; Indomethacin; Ketoprofen; Meclofenamate; Nabumetone; Naproxen; Oxaprozin; Phenylbutazone; Piroxicam; Sulindac; Tenoxicam; Tiaprofenic Acid; Tolmetin
ใช้ป้องกันอาการปวดศีรษะจากหลอดเลือด Vascular headache prophylactic ได้แก่ยาFenoprofen; Ibuprofen; Indomethacin; Mefenamic Acid; Naproxen
ใช้แก้ปวดศีรษะจากหลอดเลือดสมอง Vascular headache suppressantได้แก่ยา Diclofenac; Diflunisal; Etodolac; Fenoprofen; Floctafenine; Ibuprofen; Indomethacin; Ketoprofen; Meclofenamate; Mefenamic Acid; Naproxen

วิธีการใช้ยาที่ถูกต้องเป็นอย่างไร
หลังจากที่รับประทานยานี้แล้วท่านจะต้องทำตามขั้นตอนดังต่อไปนี้

ให้ดื่มน้ำทันที 1 แก้วและห้ามนอนราบเป็นเวลา 30 นาทีเพื่อป้องกันยาระคายเคืองต่อกระเพาะและหลอดอาหาร
ให้รับประทานยานี้พร้อมอาหาร หรือรับประทานยาลดกรดร่วมด้วย
ยาที่ออกฤทธิ์ช้า delayed-release (enteric-coated) ไม่จำเป็นต้องรับประทานพร้อมอาหาร หรือยาลดกรด
ยานี้ให้กลืนห้ามเคี้ยว

จะต้องหลีกเลี่ยงอะไรหากรับประทานยา celecoxib

หลีกเลี่ยงการดื่มสุราเพราะจะเพิ่มความเสี่ยงเลือดออกในกระเพาะอาหาร
ไม่ควรจะรับประทานยาในกลุ่มยาแก้อักเสบ NSAIDs
ไม่ควรรับประทานยาแก้ปวดหรือยารักษาไข้หวัดที่มีส่วนผสมของยากลุ่ม NSAID โดยจะต้องอ่านสลากยาก่อนรับประทาน
หลีกเลี่ยงแสงแดดจ้าๆ

หากท่านรับประทานยาดังต่อไปนี้จะต้องแจ้งแพทย์

ยาต้านการแข็งตัวของเลือด
ยาขับปัสสาวะ
ยารักษาเชื้อรา Fluconazole
ยา Lithium
ยาลดความดันโลหิต เช่น candesartan , irbesartan, losartan, valsartan , telmisartan, olmesartan

The Northern Hemisphere is once more beginning its lean towards the Sun, with the spring equinox taking place last Friday, March 20th. People all over are welcoming the spring sunshine and new growth in many ways - from Stonehenge and Mayan pyramids to Dutch meadows and Texan beaches.
